All children entering Kindergarten in the fall must have received the following immunizations to fulfill the California State Requirements:
- Three doses of Polio vaccine. Meets requirement for ages 4-6 if at least one was given on or after the 4th birthday; three doses meets requirement for ages 7-17 if at least one was given on or after the 2nd birthday
- Four doses of DPT/TD vaccine. Meets requirements for ages 4-6 if at least one was on or after the 4th birthday. Effective date: 7/1/99
- Two doses of Measles vaccine must have been given on or after the 1st birthday
- One dose of Rubella vaccine must have been given on or after the 1st birthday
- One dose of Mumps vaccine must have been given on or after the 1st birthday
- Mantoux TB skin test, plus one dose of Varicella (Chicken Pox) as of 7/1/01
- Three doses of Hepatitis B vaccine
In addition, children are required, by law, to have a physical examination within eighteen months of entrance into 1st grade. There is a health check-up form (CHDP) which must be completed and signed by a physician. This is supplied by the school at the beginning of Kindergarten, giving parents ample time to complete the check-up.
Students entering 7th grade must have had Measles and Rubella immunization and Pertussis immunization. Any students entering a California school for the first time will be required to take the Mantoux TB skin test plus Varicella. Three doses of Hepatitis B are also required for 7th grade.
