Immunization/Medical Records
Students entering our school for the first time must bring a record of the following:
Vaccine | Required Dose |
Polio | 4 doses at any age, but…3 doses meet requirement for ages 4-6 years if at least one was given on or after the 4th birthday, 3 doses meet requirement for ages 7-17 years if at least one was given on or after the 2nd birthday.

Diphtheria, Tetanus, and Pertussis | Age 6 years and under: DTP, DTaP or any combination of DTP or DTaP with DT (diphtheria and tetanus) 5 doses at any age, but…4 doses meet requirements for ages 4-6 years if at least one was on or after the 4th birthday.

| Age 7 years and older: Tdap, Td, or DTP, DTaP or any combination of these 4 doses at any age, but…3 doses meet requirement for ages 7-17 years if at least one was on or after the 2nd birthday. If last dose was given before the 2nd birthday, one more (Tdap) dose is required.

Measles, Mumps, Rubella (MMR) | Kindergarten: 2 doses both on or after 1st birthday.

| 7th grade: 2 doses both on and after 1st birthday.

| Grades 1-6 and 8-12: 1 dose on or after 1st birthday.

Hepatitis B | Kindergarten: 3 doses at any age

Varicella | 1 dose
|
Tdap Booster (Tetanus, reduced diphtheria, and pertussis) | 7th grade: 1 dose on or after 7th birthday.

St. John of God is included in the jurisdiction of the L.A. County Health Department. Immunizations according to state or county requirements are required on first admission to school. State regulations demand that each Kindergarten student and any new student have a California Immunization Record on file. In some cases, the information required on this form might be repetitious to that of the School Health Record, but it is necessary to complete both. Failure to provide the requested verification will result in your child being excluded from entering school until this requirement is fulfilled.
First graders must have a Report or Diagnostic Examination and School Certificate on file. Forms are available in the office.
Students who attended St. John of God last year need only to have cards kept current. If the student has received any immunization during the summer, or there is any additional information that should be listed on the health card, please bring dates and/or information with you when you register your child/children.
