COACHING EMPLOYMENT REQUIREMENTS All coaches must complete the following programs/paperwork and submit to Athletic Department:

• Be VIRTUS certified

• Complete Livescan Fingerprinting

• Child Abuse Form

• Play Like A Champion(PLAC) Certified

• Be certified in CPR (every two years)

• Be certified in First Aid (every two years)

• Be versed in theory and coaching techniques

• Concussion Certified

• Sudden Cardiac Arrest Certified

• I-9 and W-4 Forms

• Coaching Employment Agreement (Contract)

• TB Skin Test (Mantoux)

• Emergency Information Form

• CIF Coaches Code of Ethics

• Guidelines for Professional Relationships with Students
