5.10.6 Employee Benefits Information

​In addition to any employee benefit contribution authorizations that are maintained in the payroll file, locations must have the following available on site:

  • Summary benefits booklets (e.g., pension plan, 403(b) annuity plan)
  • Enrollment forms (blanks)

  • Claim forms (blanks)​

  • Employment Development Department disability insurance forms, for Archdiocesan Catholic Center and cemeteries and mortuaries employees
