4.13.3 CUMULATIVE PUPIL REPORT
Schools shall maintain a Cumulative Pupil Record that contains full and accurate information for each student. Information should include only the following information:
Student name, date of birth, residence address and other identifying information
Parent/guardian name, address, identifying and contact information
Sacramental information (baptism, first communion, confirmation)
Enrollment information (entrance, withdrawal, re-entrance, graduation)
Standardized test results
A transcript of classes
Attendance information
A record of the location that receives a copy of pupil records
Verification of or exemption from required immunization (recorded through high school graduation)
Cumulative Pupil Record forms are kept on file permanently as hard copies in secure fireproof containers or as electronic copies (see "Student Cumulative Records" in the Record Retention Schedule, sorted by category and record type). Only authorized personnel may have access to these records. The school must maintain health records in a separate file for four years.
