Mandatory Participation Information

To participate in athletics at BMHS, you must complete steps 1-3 listed below to be eligible to participate in sports. If the proper paperwork is not completed, the student will be removed from participation until they are eligible to participate by the athletics office. All paperwork must be submitted on a yearly basis and please do not submit hard copies of physicals or insurance cards; they must all be submitted online via https://www.athleticclearance.com/

*All 3 steps must be completed by the Summer Athletic Skills Course start date (if participating in a summer athletic course) or before the tryout of the sport the student wants to participate in.

Step 1 Online Athletic Clearance
Student-athletes and their parents must complete all steps of the athletic clearance process at www.athleticclearance.com. “How To” found below.

Step 2 Upload Physical Exam
A copy of a current physical exam must be uploaded to the online athletic clearance. Physical exams must be completed by any MD/DO. The required physical exam form can be found in the "forms" tab.

Step 3 Upload Insurance Card
A copy of the student-athletes medical insurance card must be uploaded to the online athletic clearance.
