ATHLETIC CLEARANCE PACKET AND PHYSICAL EXAMINATION
A yearly athletic clearance packet and physical examination are required. The standard physical form must be completed and submitted to the athletic department prior to participation. The examination covers all sports for the entire school year. The form will be kept in the Athletic Department.
EMERGENCY CONTACT FORM
Each Athlete's parent(s) / guardian shall complete an Emergency Contact Form attached to the physical form, giving permission for treatment by a physician or hospital when the parents(s) / guardian is not available. This form must be completed prior to participation.
