In addition to any employee benefit contribution authorizations that are maintained in the payroll file, locations must have the following available on site:
- Summary benefits booklets (e.g., pension plan, 403(b) annuity plan)
Enrollment forms (blanks)
Claim forms (blanks)
Employment Development Department disability insurance forms, for Archdiocesan Catholic Center and cemeteries and mortuaries employees
